The pre-surgery questionnaire provides the center with your health information and medical history to ensure you receive the safest care possible.
Here is what you can expect and the steps needed to complete the questionnaire prior to your surgery.
- When your procedure is scheduled, you will receive an email and/or text message with a secure link that is custom for you.
- As soon as you receive the notification, click on the link and complete the easy, online questionnaire that will include a review of your medical history, allergies and medications.
- To access the questionnaire, you will need your name and date of birth.
A registered nurse from New England Surgical Suites will contact you via phone one business day prior to your surgery to provide you with the final details of your procedure and your arrival time. If your surgery is on Monday, you will receive a call on Friday.
